As the health and social care system has moved out of the critical pandemic response, the work of the HCP has increasingly shifted away from a tactical/operational focus back to the development of a longer term vision of the partnership, and more recently developing ideas for future models of place based working in response to the White Paper: Integration and innovation: working together to improve health and social care for all.
We have come together across Sheffield to review how we work together and what we should focus on. The roles and priorities of the delivery groups below have also been reviewed to focus on the needs of our communities across Sheffield.
These delivery groups will be leading on priority areas identified in the Sheffield Place Partnership Plan 2023 – 2025 viewed here, alongside other work. By working in partnership, we aim to improve the experience of care and health outcomes for the population of Sheffield. The oversight of this work will be managed through the HCP governance structures which can be viewed here.
